Chosen patient needs to have a chronic long term condition and an acute condition For the chronic long-term condition this is to be chronic obstructive pulmonary disease and the acute condition is to be a sacral pressure ulcer.
You need to look at Activities of daily living from Roper Logan and Tillney Model.
Choose one aspect of care to write about (chronic obstructive pulmonary disease) and say how it has impacted on the patient and us as nurses looking after them.
Look at standard nursing text on complex needs.
Select a patient/client with complex needs with whom you have been involved in providing care. Through the use of a systematic approach you should assess the individual’s health needs and consider a range of factors contributing to the complex health breakdown. From these identified needs select one aspect of care and provide a rationale for this selection. Provide a critical reflection of the relationship between goals set and care delivered. In relation to the chosen need recommendations for practice made should be discussed.
The analysis should integrate relevant knowledge from nursing (e.g. care delivery system), confidentiality using the nursing and midwifery Council code of conduct, the behavioural sciences (e.g. power and status) and biological sciences (e.g. stress).
