Write a summary that provides a critical analysis of the major issues affecting your community’s health, with particular relevance to your aggregate assigned to you or your clinical group. The emphasis should be on writing a quality report that highlights your ability to identify and critically analyze the raw data make recommendations for your community. This section should be6 pages in length. Use 12 font Times Roman font, 1-inch margins, and double- spaced following APA format 6th Edition only. Marks will be deducted for incorrect APA format.
Steps for preparing your critical analysis summary:
a) Write a concise Introduction and Purpose: what is a community assessment, why is it important in the community health nurse’s assessment role and why you selected to the assessment in that particular community/neighbourhood where your placement site is located. Describe here who your aggregate is.
b) Data Collection Methods: explain what were the steps in your community assessment.
c) Key Informants: who did you consult and why did you select these individuals/groups.
d) Summary of Findings: identify 5 key social determinants of healththat impact your aggregate. ( Income and social status, education, environment, social network and immigration) Summarize the key demographics; assets and capacities; barriers; vulnerabilities and issues of quality of life in the broader target community.
e) Preliminary Recommendations: address health issues in your aggregate and provide recommendations. While you or your praxis group may not be able to address these recommendations due to time restraints, a community health assessment nevertheless requires recommendations to address these in future.
f) Next Steps:which of the above recommendations should the community focus on and how a community health nurse can be an integral part of this recommendation. Use 1-2 examples of health initiatives that may relate to these recommendations.
